A Rural County Waiver for 120 beds had been in place for five years to build a new Skilled Nursing Facility in a Texas coastal community without being used.
The bed allocation had been encumbered by an illegal option placed against the beds which prevented their use. Several groups had attempted to build but were not successful because of the encumbrance.
Cole Consultants brought together state regulators, the county judge, the owner of the bed allocation, the buyer and the hospital board of directors and resolved the issue by getting the option removed from the waiver allowing construction to begin.

Acting on a request from the U.S. Department of Housing and Urban Development to remedy a problem with a distressed property, Cole Consultants found a qualified buyer for the Medicaid bed allocation of a closed nursing home in Austin that had been abandoned for over two years.
A North Texas rehab hospital was denied their Medicare approval for a new distinct-part Skilled Nursing Facility unit in their hospital, jeopardizing the entire project after months of work and a significant financial investment. Cole Consultants was able to have the denial reversed and Medicare approval granted within three days, enabling our client to reach their goal of expanded service to the community and increased income for the hospital.
A review by Cole Consultants of a client’s 10 percent Medicaid bed increase request revealed a mistake by DADS that would have resulted in a loss of three months of income from the beds.
The mistake was corrected and the loss was averted.
After their initial request for a license was denied, the first-time owners of a Dallas Adult Day Care Center hired Cole Consultants to guide them through the licensure and certification process, the start-up of the business, and staffing issues which resulted in the successful opening of the facility.
Six of the seven Rural County Waivers currently pending certification by the Texas Department of Aging and Disability Services (DADS) were secured by Cole Consultants for various clients around the state thus enabling new nursing homes to be built in communities where they are most needed.
